Well, it probably will - even if the basement is only placed under the otherwise unchanged house. It should be accessible from the house, and it must therefore also be decided whether it should become part of the thermal envelope and so on. A basement is not a pantry or dressing room door.

Why does the floor plan have to change? The utility room could become an office/guest room. The upper floor could stay like this.

What would you do in my situation?

I can hardly work with a level now. A neighbor is already sitting above street level (with basement).

In the present tense, we would not be in your situation. In the past tense, when you were tormenting the poor draftsman with your various princess peas, we would not have introduced the information about the unevenness of the property into the discussion so late, and of course we would have dealt with such fundamental decisions at an earlier stage. You don’t just plan a basement on the fly with a plasterboard adjustment, like you would shift an interior door lintel by one stone width.

I’m going to the post office briefly. In the meantime, you can think about how that would sound as a clear sentence.

There could be certain specifications in the development plan, e.g. number of floors. The architect should actually be aware of this.
