I don’t know what experiences you have had with the tax office. But there, an immediate repayment and a fine are probably the least severe penalties. There can be a report for tax fraud.

It was clarified with the building authority. Accordingly, we must be able to designate two parking spaces for the "house."
Not two per residential unit. Therefore, the design here would be sufficient.

We initially also thought that we had to build barrier-free in the basement. However, this is not a requirement for a later practice. We were also surprised by this, but it works in our favor in this case. If we rename a "granny flat" to a "practice" afterwards, I have not read anything to the contrary yet.. I will double-check here again and ask this question directly at the KfW. Thanks for the tip.

You have to officially register the rooms for use as a practice as commercial space – and this usually involves providing proof of parking spaces. I believe that for appointment-only practices without "walk-in patients," there are 2 parking spaces that must also be marked and kept free.

An acquaintance of mine has set up a small hair salon in the basement. I am currently wondering whether similar considerations regarding parking spaces etc. have also been made there. Nothing is specifically designated.

You want to build in Bavaria, right? The building code clearly prescribes barrier-free construction for practice rooms. BayBO Art. 48 Barrier-free construction.
