The difference is as follows:
An air-to-air heat pump extracts heat from the air (outside air or exhaust air) and transfers it to the interior spaces via ventilation. Underfloor heating or radiators are not needed.
An air-to-water heat pump transfers the heat from the air to a heating circuit with underfloor heating or radiators.
