A pharmacy cabinet is of course much more expensive than the basic frame for a refrigerator conversion because it has many pull-outs. Pull-outs and drawers are probably the most expensive part of a kitchen (apart from the electrical appliances).
In the kitchen planner, you can find both those for built-in electrical appliances under "Tall units" and those with pull-outs, once as a pharmacy cabinet and once as a cabinet with a hinged door and pull-outs behind it.
Once you understand the system, it is really easy to plan a kitchen with it. However, the planner is not suitable for special solutions and extra requests; you probably have to clarify that on-site at Ikea.
