If it starts off like that... Moisture can get in. Then it begins to rust, and as a result, the concrete can crack. We had a similar problem on the upper floor. Due to too many pipes, we had to chip away some of the ceiling at the intersections. Put a few pipes in and re-concreted it. This was particularly important to our site manager and he emphasized it very often.

Yes, this is certainly a defect. The concrete cover of the reinforcement is clearly incorrect here, as it is not inside but almost on top. Either the slab is too thin (concrete amount based on the old calculation, subsequently required to be thicker) and/or the reinforcement is installed too high (incorrect spacers). The notion that the screed would somehow fix this or that it would be acceptable relative to what is built on top is, with all due respect, a joke comment. I see at least the need for expert evidence collection. This is a violation of DIN and aRdT, which cannot be dismissed as a cosmetic defect. I would also view this as an indicator to question the suitability of the workers.

Above all, the insulation comes first, not the screed.

No, first a separating layer is applied, for example bitumen.
If you want to be a smart aleck, then please do it right...

I'll put it quite directly, we had "only" 3 small spots on our floor slab at the edge (not on top) where the reinforcement was minimally visible. Our accompanying construction inspector immediately criticized this, which resulted in the 3 small spots being chiseled out and sealed again with a special concrete sealing compound. Your open spots are a serious defect that must be professionally corrected, no ifs or buts.
