You are addressing one of the core problems. This is also why involving an expert is so important. Most errors in execution can be detected at a very early stage and can then be corrected (because otherwise, the contractor, in the best case, does not get paid). If the faulty executions accumulate on a contractor's sites, then insolvency is THE way to rid themselves of these burdens and start over in another form (there are so many aunts, uncles, friends, etc.).
