I would access the utility room from the hallway. Not only does the door limit the furnishing in the bathroom, it is uncomfortable since the bathroom is considered a passage area - also practically. If not in the bathroom, where else can you relax undisturbed?! Besides, it makes it independent. If someone wants peace while showering or bathing, someone can still always get to the laundry. Also, you don't need to go into the utility room every time, but only occasionally. There is no reason not to have the door from the hallway.

I would not store medication in the same room as the dryer because of the temperature.

Yes, I am also shocked that you want to go through the bathroom first. That would be a last resort if there is no other way. But not if you can enter from the hallway.

So, utility room door to the hallway and then renovate the bathroom. There are several possibilities. Here are two plans sketched out wildly. Maybe others have more ideas.

We also have the door in the bathroom. Not a makeshift solution. It's perfect. The door probably opens and closes 20 times a day and each time it's just one step.

?huh? To get to the utility room, it's not just one step. You first have to go through the bathroom. And if you have to go from the bathroom to the utility room 20 times, then in my opinion that's a planning error, because you have something in the utility room that actually belongs in the bathroom.

The utility room is only accessible through the bathroom and should be at least "separate". I wouldn't want that. I know such solutions from houses that have been extended from time to time.

I would prefer a wide sink where two people can stand to two separate ones.
A privacy screen is basically OK - is that the only toilet in the house?
There are shower-bath combinations with a low entry that save space. Maybe that would be a space-relieving alternative.
