To explain the representation:
Arrow pointing upwards - pipe goes up through the floor slab.
DD = ceiling breakthrough 18/18 = dimensions of the breakthrough.
So here, a pipe starts from this floor and goes upwards through the floor slab via the opening in the ceiling.
This will actually be the ventilation, as this is stated in the explanation for DD.
If an arrow pointing downwards is drawn, it means that a pipe starts here and goes downwards.
If the arrow does not break through the circle, it is a pure starting point of a riser or a pipe. Then there is usually no DD located there.
For S5, I would assume that S stands for riser and all riser pipes have been numbered.

I see it differently.
The symbol here is a continuous riser. It comes from the floor below and goes to the floor above. The symbol starts on the left below the breakthrough, which you described correctly.
I always wonder what some heroes in my profession draw. Primarily, this is probably just a soil stack fall pipe, which then vents upwards above the roof. If this is the top floor, the following symbol would be more appropriate and understandable.
