Bafa funding application form lost

Hello everyone,
we had an air-to-water heat pump installed in a house from 1980. We submitted the application in advance to Bafa, got the application approved by Bafa, and had the air-to-water heat pump installed. Everything went according to plan. Until now. Unfortunately, we have now misplaced the form "Bestätigung der wahrheitsgemäßen Angaben". Without this form, it will probably not be possible to receive the funding payment. Unfortunately, no one is reachable by phone at Bafa. This form cannot be found online either. We are now a bit at a loss. Has anyone had a similar experience here and has a tip for us [Pappnasen]? Thanks in advance and best regards.

the first link on Google is immediately a PDF file from the Federal Office for Economic Affairs and Export Control (BAFA), namely the "Merkblatt zur Antragstellung" ... and there on page 11 could be your missing form - right?
