Or the exhaust air should have been led directly through the outer wall (available?) instead of through the roof (as decentralized ventilation with heat recovery, that still works now).

that will work because the pipe will warm up due to the constant warm airflow and stay warm through the insulation. therefore, there will be no more condensation.
