Underlay membrane moisture and crack

Hello everyone,

I have discovered moisture in our underlay membrane and while searching for the source I came across the following (see pictures below).

Does anyone have an idea where this comes from, whether this is supposed to be like that or intended, or if there is a damage to the underlay membrane.

If there is damage, how should it be properly sealed? This extends over several rafter fields and it looks like a sheet metal is sticking out there.

Please show that to the carpenter or roofer. That looks abnormal.
