Building description on official form § 5 para 1 Building Inspection Regulation

can someone tell me which form this is supposed to be?

When I search for it, I usually end up with a form for the simplified approval procedure. Sometimes there are also hits on the topic of special buildings.

Is the linked form what I need according to § 5 Abs 1 BauPrüfVO?

Thank you. I took a look at it and find it a bit strange.

We are currently filling out the form for approval exemption (§67). We state: Further processing as a building application. At No. 2.4 it then says: Building description on official form (§5 para.1 BauPrüfVO).

What you have to fill in the building description sounds to me more like information for larger, public buildings. But if that's how it is supposed to be, then we'll just do it...

I saw that Kerpen is written on the edge - is there a neutral NRW template, does each municipality have a different one, or does it not matter that X is on the edge and I submit it to Y?
